Summer Camp Scholarship Request

I, the undersigned parent/guardian in consideration of the instruction given to said child/ward at the summer history day camp, herely release and discharge the Rowan Museum, Inc., it’s officers, instructors, and employees, from any and all claims, demands or cause of action I may have by reason of illness, injury or accident incurred or suffered by said camper while attending said summer history camp.
